Integrated care seminar: Prevention and the patient pathway

The Accountable Care Journal (ACJ) welcomes you to attend our thought-provoking seminar in the heart of Sheffield to explore the challenges of long-term conditions, lifestyle diseases and the social determinants of health, from the perspective of the patient pathway. Our seminar is sponsored by legal experts Browne Jacobson. ACJ seminars offer attendees an exclusive insight to the leading developments in integrated care – spanning health, social care, public health, local government and social services – while getting the opportunity to engage with other leading stakeholders keen to make their ideas and concerns known at this critical time of health service reform. Healthcare is becoming increasingly patient-centric as the national policy focus shifts to look at prevention and early intervention strategies. However, this has to be delivered for local populations in local areas.  Reforms are underway in England and health and care professionals need to know what's working. Following the publication of the NHS Long Term Plan, the attention of health and care policy has shifted all the more substantially to focus on keeping people well, as opposed to simply treating the sick.  This is combined with new challenges, growing health inequalities, an ageing population which brings greater prevalence of multi-morbidity and long-term conditions, and more people suffering from lifestyle diseases. We are delighted to bring together Gerard Hanratty, Partner at Browne Jacobson, Dr Andrew Hilton, Chief Executive of Primary Care Sheffield, Professor Dean Fathers, Chairman for Nottingham Healthcare NHS Trust, Keith Strahan, Principal Clinical Lead at NHS Digital, Mindy Daeschner, Chief Commercial Strategy & Marketing Officer at DoctorLink, Dr Trevor Smith, Consultant Gastroenterologist at BAPEN will be joining our keynote speaker, Dr Charles Alessi, Chief Clinical Officer for HIMSS International, on Tuesday 7th of May in Sheffield. The seminar will look at the patient pathway, how services cooperate and interlink to treat patients and what is being done to keep people healthy.  What are the biggest challenges to good population health today? Are the usual public health offenders still at large or are new issues on the horizon? How can lifestyle changes reduce the burden on overstretched health and care services? How can we improve leadership, relationships and workplace culture in health and care?  What does the future look like for prevention and addressing health inequalities in England? What does primary care reform have to offer and how is integration helping to address the issue?  How is this all framed in terms of the Long Term Plan, system reform and integrated care?  Does this offer a light at the end of the tunnel for workforce woes?  In the fast-paced and ever-changing landscape nationally, it is more important than ever that these questions are addressed with local populations in mind.Agenda and speaker line up:Welcoming tea's and coffee's: 9:30am - 9:50amChairs Welcome: 9:50am - 10:00am Keynote speaker's session: Dr Charles Alessi, Chief Clinical Officer - HIMSS: 10:00am - 10:40am (Shared decision making in the era of precision medicine - from personalised risk reduction to the potential to better manage clinical decision support) ACJ Partners session: Gerard Hanratty, Partner - Browne Jacobson: 10:40am - 11:30am (Sponsoring Session) 1st Break: 11:30am - 11:45am 1st Early afternoon session: Professor Dean Fathers, Chair - Nottingham Healthcare NHS Foundation Trust: 11:45am - 12:15pm (Democratisation of Healthcare)2nd Early afternoon session: Keith Strahan, Principal Clinical Lead - NHS Digital: 12:15pm-12:45pm (Integrating Health & Social Care)3rd Early afternoon session: Dr Trevor Smith, Consultant Gastroenterologist - BAPEN: 12:45pm - 13:15pm Lunch break: 13:15pm - 14:15pm (Effective Menagement of Malnutrition in Health and Social Care Improves Lives & Saves Money)1st Afternoon session: David Rogers, Chair - North Staffordshire Combined Healthcare NHS Trust: 14:30pm - 15:00pm (Committing to patient centred care: Implications and outcomes)2rd Afternoon session: Primary Care Panel Discussion: Dr Andrew Hilton, CEO - Primary Care Sheffield.
